Payment at time of services, but superbill will be provided to submit to insurance for possible reimbursement.
Cost is $225 for in home visit and includes evaluation, prescription call in as needed, work note. If IV treatment warranted, additional charges apply.
Cost includes 10 mile radius from office, if farther, $1/mile additional.
